VII. REPORT A LOST OR STOLEN PURCHASING CARD
 
It is the responsibility of the cardholder to report the loss of a Purchasing  Card immediately to (1) U.S. Bank, NA and (2) UNL's Purchasing Card Program  Administrator in the E-Commerce Department.

If a Purchasing Card is lost or stolen, the cardholder MUST IMMEDIATELY  CONTACT U.S. BANK, NA.  U.S. Bank, NA representatives are available 24 hours a day, seven days a week. When reporting a lost or stolen card, the cardholder  MUST tell the representative the call is regarding a University of Nebraska Purchasing Card.

After reporting a lost or stolen card to U.S. Bank, NA, the cardholder MUST ALSO IMMEDIATELY REPORT A LOST OR STOLEN CARD TO THE UNL PURCHASING CARD PROGRAM  ADMINISTRATOR in the E-Commerce Department at 402-472-5613 or 402-472-3330.
